Why a Breast Revision

Breast revision surgery is a powerful way to refine your appearance after an augmentation or address changes that have developed over time.

Whether you’re seeking to correct implant-related concerns like capsular contracture, rupture, or shifting, or simply want to update the size, shape, or type of your implants, this procedure offers a path to renewed confidence. Many women also choose revision to restore their silhouette after pregnancy, weight fluctuations, or aging, ensuring their results align with their current goals and lifestyle.

The core aspect of breast revision for breast cancer involves removing mass or masses, which frequently leaves scars and breast deformities. There are several reconstruction options that can preserve the breast tissue in the setting of lumpectomy or restore breast contour if a mastectomy is required.  Breast reconstruction may require several stages spaces several months apart.

Our doctors have extensive experience and training with oncologic breast reconstruction, implant-based reconstruction and free tissue or microsurgical breast reconstruction. The primary goal is to ensure you eradicate cancer and then secondarily we can restore your breast contour and feminine form.

Breast Implant Exchange at Quintessa Aesthetic Center

Breast implant exchange is a straightforward and effective procedure for patients who wish to change their breast size, update their implant type, or address implant-related concerns. Whether you’re hoping to go bigger or smaller, or simply want a different look or feel, Dr. Butz and Dr. Thayer at Quintessa Aesthetic Center offer personalized care to help you achieve your goals.

Patients often consider implant exchange for a variety of reasons, including:

  • Wanting larger or smaller implants to better suit their current body or lifestyle
  • Experiencing changes in breast shape or position after pregnancy, weight fluctuations, or aging
  • Correcting complications such as capsular contracture, implant rupture, or malposition
  • Upgrading to newer implant types or profiles for a more natural look or improved comfort

In most cases, Dr. Butz or Dr. Thayer will use your original incision lines to remove the old implants and place the new ones, minimizing additional scarring. If you are changing to a significantly smaller size or if there is excess skin or tissue laxity, a breast lift may be recommended to ensure the best aesthetic result.

Recovery after implant exchange is typically similar to, or even easier than, your initial augmentation. Most patients can return to work and normal activities within a week, while strenuous exercise should be avoided for several weeks as you heal. Because the implant pocket already exists, healing may be quicker and more comfortable for many patients.

At Quintessa, Dr. Butz and Dr. Thayer are committed to helping you feel confident and comfortable throughout your implant exchange journey, ensuring your results align with your current goals and lifestyle.

Addressing Capsular Contracture

Capsular contracture—a tightening of scar tissue around breast implants—can cause firmness, discomfort, or misshapen breasts. At Quintessa, Dr. Butz and Dr. Thayer specialize in corrective breast revision surgery to alleviate pain and restore a natural appearance. During the procedure, hardened scar tissue is carefully removed, the implant pocket is sanitized to reduce infection risk, and new implants are placed with precision. For recurring cases, advanced techniques like submuscular placement or acellular dermal matrices (e.g., Strattice®) may be used to minimize recurrence. Whether addressing implant displacement, discomfort, or aesthetic concerns, our goal is to provide lasting relief and results that align with your vision of confidence and comfort.

Implant Malposition

For patients experiencing implant malposition (bottoming out)—where breast implants shift from their intended position due to factors like pocket overdissection or tissue stretching—advanced surgical techniques such as pocket modification, internal bra mesh reinforcement, and subfascial conversion offer tailored solutions to restore symmetry, comfort, and aesthetics.

Internal Bra Mesh Reinforcement

Internal bra mesh (e.g., TIGR®, GalaFlex®, or Durasorb®) provides structural support to weakened tissue, addressing issues like bottoming out (inferior malposition) or lateral displacement. This technique involves:

  • Reinforcing the implant pocket by suturing the mesh to the chest wall or existing breast tissue, creating a stable “internal bra” to hold implants in place.
  • Correcting weakened inframammary folds (IMF) by restoring anatomical boundaries and preventing recurrent sagging.
  • Combining with capsulorrhaphy (capsule tightening) for a “belt and suspenders” approach to ensure long-term stability.

Mesh is particularly beneficial for patients with thin tissue, recurrent malposition, or prior complications like capsular contracture. Studies show it reduces recurrence rates by up to 89.9% when paired with precise pocket adjustments.

Subfascial Conversion

Subfascial conversion is a modern technique that involves relocating implants from a subpectoral to a subfascial plane (beneath the fascia but above the muscle). This technique is ideal for:

  • Correcting animation deformity (implant movement during muscle contraction) by eliminating pectoralis major influence.
  • Resolving inferior/lateral malposition caused by submuscular pocket instability, as the fascia provides natural support without muscle tension.
  • Avoiding IMF disruption in cases of double-bubble deformity, where subglandular or subfascial placement redistributes pressure more evenly.

Combined Approaches

Every patient is unique and during the consultation Dr Butz or Dr. Thayer will come up with a customized plan to help correct your concerns.

  1. Convert the implant plane (e.g., subpectoral to subfascial) to eliminate distorting forces.
  2. Layer mesh support to reinforce the new pocket, particularly in high-risk cases.
  3. Perform concurrent mastopexy to address skin laxity and optimize nipple position.
  4. Fat Grafting to help improve contour abnormalities

By tailoring techniques to each patient’s anatomy and goals, surgeons can achieve durable, natural-looking outcomes while minimizing recurrence risks.

Breast Implant Removal at Quintessa Aesthetic Center

Choosing to remove breast implants is a personal decision, often made for a variety of reasons such as discomfort, changes in lifestyle, complications like capsular contracture, or simply a desire for a more natural look. At Quintessa Aesthetic Center, Dr. Butz and Dr. Thayer provide compassionate, individualized care for women considering breast implant removal, also known as explant surgery.

During your consultation, Dr. Butz or Dr. Thayer will discuss your reasons for removal and help you understand the best surgical approach for your needs. Implant removal may involve taking out just the implants or, in some cases, removing the surrounding scar tissue (capsule) as well.

The procedure is typically performed as an outpatient surgery, using incisions either along the lower fold of the breast or around the areola, depending on your previous surgery and anatomy. If desired, a breast lift can be performed at the same time to address any stretched or sagging skin and restore a more youthful breast shape.

Breast implant removal can address issues such as implant rupture, hardening of scar tissue, discomfort, or aesthetic concerns. Dr. Butz and Dr. Thayer are dedicated to ensuring you feel informed, supported, and confident throughout your journey, from your initial consultation to your recovery.

At Quintessa, your safety, comfort, and satisfaction are our highest priorities. If you are considering breast implant removal, our board-certified plastic surgeons are here to guide you through every step with expertise and understanding.

Our Three Step Process

1-
The Breast Revision Procedure

Before undergoing breast revision surgery, you will consult with our very own Dr. Daniel Butz or Dr. Jacob Thayer, our board-certified plastic surgeons with extensive training and experience with breast-related treatments and procedures. They will take as much time as needed to do a full examination of your breasts in order to determine the best revision process for your unique situation.

During your breast revision surgery, our doctors will closely follow your unique revision plan. Your new breast revision will allow you to regain and maintain the youthful look you have always wanted.

2-
Breast Revision Recovery

Recovery time following a breast revision surgery depends on the extent of the procedure. Most patients cover from a breast revision surgery in a week.

3-
Postoperative Planning

The procedure location depends on the procedure being performed. The mastopexy and first stage of reconstruction is an inpatient procedure but the subsequent stages can be performed as an outpatient. A dressing will be in place when you wake up and this will be removed by our doctors the following day in the clinic. You will then be placed in a soft bra. You should avoid underwire bras for at least 4 weeks. You may have some numbness and hypersensitivity of the breast. This is normal and will improve with time. You should avoid rigorous exercise for at least 2 weeks but we encourage you to be up and walking several times throughout the day.

Frequently Asked Questions:

What are the common reasons patients seek breast revision surgery?

Patients often pursue breast revision surgery to address implant rupture, hardening of scar tissue around the implant, dissatisfaction with the size or shape of the implants, or changes in breast appearance due to aging, weight fluctuations, or pregnancy.

Unlike the initial augmentation or reconstruction, breast revision surgery focuses on correcting or enhancing previous outcomes. This may involve removing or replacing implants, adjusting implant placement, or addressing complications like scar tissue formation.

The specifics of a breast revision procedure depend on individual needs. Generally, it may include removing or replacing implants, reshaping breast tissue, or correcting issues like asymmetry. The surgeon tailors the approach to achieve the desired aesthetic outcome.

The complexity of the correction affects recovery. At first, patients may experience some discomfort, bruising, and swelling. Within a week, most people resume light activities, but for best recovery, it’s crucial to adhere to the surgeon’s post-operative care guidelines.

A comprehensive meeting with your surgeon to go over your objectives and medical background is part of the preparation process. To encourage better healing, you might need to stop smoking, modify your present medications, and have medical exams. It’s also a good idea to arrange for help while you’re recovering.
