Breast Reduction Surgery: What to Expect Before & After

You may have spent years adjusting bra straps, avoiding certain clothes, or planning activities around neck, shoulder, and back discomfort. When your overly large breasts limit how comfortably you move through daily life, breast reduction can feel less like a cosmetic decision and more like a step toward feeling at home in your body again.

At Quintessa Aesthetic Center, board-certified plastic surgeons Dr. Daniel Butz and Dr. Jake Thayer provide personalized breast reduction surgery for patients throughout Wisconsin. In this article, we help you understand what to expect before and after surgery.

What to expect before breast reduction surgery

A consultation centered on your experience

Your concerns and goals guide the conversation. You may want relief from physical discomfort, greater freedom during exercise, improved proportions, or clothing that fits more comfortably. Your surgeon will listen to what has been bothering you, review your medical history, examine your breasts, and discuss a realistic surgical plan.

Breast size is only part of that plan. Your surgeon also considers your frame, skin quality, breast shape, nipple position, and proportions. Because bra cup sizing is not standardized, surgery cannot guarantee a particular cup size. The goal is to create a balanced result.

Medical preparation for surgery

You may need bloodwork, medical clearance, or breast imaging based on your age and health history. Tell your surgeon about every prescription, medication, and supplement you take. You will receive specific instructions about which products to pause and when.

Nicotine interferes with blood flow and healing, so you must follow your surgeon’s instructions regarding smoking, vaping, and other nicotine products. You should also arrange transportation home and organize help for the first several days.

A personalized surgical plan

Breast reduction is typically performed under general anesthesia. Your surgeon removes excess breast tissue, fat, and skin before reshaping the remaining tissue and repositioning the nipple and areola. The precise technique depends on the amount of reduction and lift you need.

Incisions commonly extend around the areola and down the lower breast. An additional incision along the breast crease may be necessary. Scars are permanent, although they soften and fade with time. Your surgeon will explain their expected locations before surgery.

Practical planning at home

Preparing your recovery space in advance can make the first week easier. Place frequently used items within comfortable reach, prepare simple meals, and choose loose clothing that opens in the front. You may need help with children, pets, shopping, driving, and household tasks. Your surgical team will also explain when to stop eating and drinking, what to wear on surgery day, and which medications you may take.

What to expect after breast reduction surgery

Rest and support during the first week

Swelling, bruising, tightness, and soreness are normal during early recovery. You must wear a supportive surgical bra, and some patients may temporarily have drains. Short, gentle walks are encouraged, but you will need to avoid lifting, reaching, pushing, and pulling.

Follow your instructions for bathing, incision care, medications, and sleeping position. Contact Quintessa promptly if you experience worsening redness, unusual drainage, fever, shortness of breath, or sudden one-sided swelling.

A gradual return to your routine

Many patients return to non-strenuous work within one to two weeks, although your personal timeline may differ. You may begin feeling more like yourself before your incisions are ready for normal activity, so resist the temptation to do too much too soon.

Your surgeon will gradually clear you for exercise, heavier lifting, and underwire bras. Continue wearing the recommended support garment and attend every follow-up appointment.

Results that settle over time

You may notice an immediate difference in breast weight and proportion, but swelling can temporarily affect shape and symmetry. Your breasts will gradually soften and settle over the following months as your scars mature.

Sensation in the breasts or nipples may temporarily increase or decrease, and permanent changes are possible. Breastfeeding may also be affected, so discuss future pregnancy and nursing plans during your consultation. Weight fluctuations, pregnancy, and aging can change your results over time.
